More Cancer Reporting

Information for physicians, ambulatory care centers, radiation treatment facilities:

Physicians, ambulatory care centers and radiation treatment facilities are required to report all non-hospitalized cancer cases to the NJSCR.  All cases must be reported within six months of diagnosis.  A separate case report form must be completed for each primary. The Cancer Registry Statute [pdf 266k] contains a copy of the reportable list. If you have never reported cancer cases to the NJSCR or your practice has just opened in New Jersey, please complete the Inventory Checklist (CES-7) [pdf 15k] so that we may learn more about your practice.  You may fax this checklist to 609-588-3638.

NJSCR Abstract Instruction Manual For Physicians, Ambulatory Care Centers & Radiation Treatment Facilities 2011 [pdf 255k]

Electronic reporting is strongly recommended. Rocky Mountain Cancer Data Systems Software (RMCDS) is available free of charge for reporting cancer cases to the NJSCR electronically. NJSCR staff will assist in the installation and provide training. RMCDS can be run on a free-standing computer or a network. This software is user-friendly and can produce patient listings, standard summary reports, statistical reports, such as frequencies, survival reports, and cross tabulations which may be useful in tracking patients in your own practice.
