This site provides the names, addresses, licensure expiration dates, and other information for the facilities which are licensed by the Department of Health and Senior Services or Federally Certified one room ambulatory surgical centers.

You may also search the Long Term Care facilities database for nursing homes and other long-term care facilities.

NOTES

#1 Home Health Agency search by county will only yield facilities that are physically located in a specific county. In order to locate all counties served by specific home health agencies, click on the facility listing.

#2 Ambulatory Care searches will not include the ambulatory care services that are provided by hospitals and listed on the hospital's license. Only services licensed to free-standing ambulatory care facilities will be included.

#3 The Department of Human Services, Division of Addiction Services licenses all residential and most ambulatory substance abuse treatment facilities. This search will yield information about Drug Abuse Treatment facilities which are Hospital Based and Ambulatory Facilities with additional services.
